Director of Care Management

Job Description

Our client, a 25-bed critical access hospital, seeks an experienced Director of Patient Care Management to join their organization. This role is responsible for providing leadership, operational oversight and direction for all aspects of case management, utilization review, and social services

 

Ideal Candidate:

 

The ideal candidate will be a highly experienced case management professional with a proven track record in performance improvements.  The director will be an effective change agent, metrics driven, understanding and supportive.  The director will build trusting and collaborative relationships with leadership, colleagues, and subordinates.  

 

Requirements:

 

  • Bachelor's Degree in Nursing.  Master’s degree in relevant field preferred.
  • Minimum 3-years of relevant experience.  Experience with case management, utilization review, and/or social work.
  • Previous experience in a supervisory/leadership role.
  • Registered Nurse must be licensed in the State of New Hampshire or hold a nurse licensure compact (NLC).
  • Case Management Certification (CCM) preferred.
